  • The award you are selecting the nominee for
  • The nominee's name, organization, job title, email address, and employment status
  • Your name, organization, job title, and email address
  • A short personal statement of why you believe the nominee deserves the award
  • A 100–200 word summary of the nominee's background, roles, responsibilities in the area of standardization
  • 1–3 letters of support
  • Curriculum vitae or resume (optional)
